(master occupational accident report form), which has been in force since 1979 and is compulsory for use throughout the country. The data contained in the forms are derived from reports concerning employee injuries filed by enterprise administrators and from medical reports submitted by medical treatment providers. These forms are then processed by two institutions: the Direction General de Hygiene, Seguridad Ocupacional y Bienestar
(General Directorate of Health, Occupational Safety and Well-being)of the Labour Ministry, which records, calculates and checks the accuracy of the data reported, and the Caja Nacional de Salud, Departamento de Medicina Laboral, of the social security system, which compiles statistics relating to all reported accidents.
